MerchantBridge Advisors
Payment Processing for Licensed Online Pharmacies
MerchantBridge Advisors reviews potential processing options for licensed online pharmacies, primarily in the United States.
Tell us about the dispensing business, products, states served and payment needs. Pharmacy dispensing is reviewed separately from telehealth; product eligibility, approval and terms require individual partner assessment.

Which entity dispenses, which prescribes and which charges?
Map the customer journey without patient details. Identify the website operator, any clinical service, the dispensing pharmacy, the business receiving each payment and the party responsible for support or refunds. A shared brand or checkout does not mean those entities perform the same regulated activity.
Separate consultation fees, pharmacy sales, memberships, delivery charges and software fees. If one business collects on behalf of another, describe the arrangement accurately. A provider willing to review a clinic does not thereby approve a pharmacy, and a software account should not conceal the collection of medication payments.
The first inquiry should explain the existing or proposed operation and the change needed. It might concern a dispensing website, a new selling channel or a different payment arrangement. There is no need to submit prescriptions, clinical questionnaires or customer order histories to explain these roles.
How should licensing and destination states be documented?
FDA's BeSafeRx guidance points consumers toward pharmacies with appropriate state licensing, a prescription requirement and access to a licensed pharmacist. For a business review, make the actual licensed entity and pharmacist contact arrangements identifiable; a professional-looking website is not a substitute for verifiable credentials. U.S. Food and Drug Administration: BeSafeRx: Frequently Asked Questions
Prepare a state-by-state operating summary covering the pharmacy's location, permitted destinations, relevant licenses and the person responsible for checking current status. Separate markets already served from proposed expansion. Do not describe a home-state license as nationwide permission or assume an online storefront removes destination-state obligations.
For a current state example, California's Board of Pharmacy states that requirements effective July 1, 2026 add California-licensed pharmacist-in-charge responsibilities and training for nonresident pharmacies serving California. Check the complete requirements for the actual operation. This is a California example, not a universal rule for all states. California State Board of Pharmacy: New Requirements for Nonresident Pharmacies, effective July 1, 2026
Summarize any material changes in ownership, location, websites or professional responsibility. The appropriate pharmacy advisers should determine what permissions those changes require. MerchantBridge's processing discussion does not make a licensing determination.
How do NABP Healthcare Merchant and Digital Pharmacy accreditation differ?
NABP describes Healthcare Merchant Accreditation as a program available to a broad range of healthcare and related businesses, with ongoing monitoring. Its stated uses include requirements of advertising platforms and card networks. That is accreditation-program information, not a promise that an acquirer will accept a particular pharmacy. National Association of Boards of Pharmacy: Healthcare Merchant Accreditation
Digital Pharmacy Accreditation is a separate NABP program for eligible licensed pharmacies with an interactive online pharmacy component. NABP requires Healthcare Merchant Accreditation as a prerequisite. The two names should not be used interchangeably, and a website badge should match the program and entity actually accredited. National Association of Boards of Pharmacy: Digital Pharmacy Accreditation
Ask the prospective payment partner which current certification or accreditation evidence it requires for your exact model and websites. Do not assume that an alternative credential is automatically interchangeable or that completing a program establishes approval for every product, state or payment function.
For preparation, list the program name, covered business and domains, current status and any pending application. Distinguish an application from an awarded accreditation. Keep the source verification available without sending private patient or prescription material in the initial inquiry.
Which products require a separate eligibility discussion?
Describe the actual pharmacy catalog and dispensing model, including any activity that differs from ordinary dispensing of approved prescription products. Identify OTC merchandise, specialty handling, compounding or other distinct operations explicitly. Do not assume a general pharmacy conversation confirms acceptance of every product or service.
FDA explains that compounded drugs are not FDA-approved and are different from approved generic drugs. Its current guidance also distinguishes compounding settings and their legal conditions. A licensed pharmacy or clinician's involvement does not turn a compounded product into an FDA-approved medicine. U.S. Food and Drug Administration: Compounding and the FDA: Questions and Answers
No compounding pathway is promised here. If compounding is part of the business, describe it for separate legal, product and payment-partner assessment. Avoid relying on an old shortage announcement, another seller's marketing or a general healthcare credential as proof that a specific product can be offered or processed now.
The business should be able to explain how product approval status, sourcing and marketing claims are checked. This is a request to describe responsible business processes, not medical advice or a recommendation of any medication.
When does a payment become a completed pharmacy order?
Map prescription receipt, professional review, stock confirmation, payment, dispensing and delivery as separate steps. Explain what happens when an order cannot proceed, including who communicates the decision and how any collected amount is handled. Do not describe a successful card authorization as a clinical or dispensing decision.
For refills or memberships, distinguish permission to charge from the clinical and pharmacy requirements for dispensing. Describe how changes, failed payments and cancellation are coordinated between the responsible businesses. A stored payment credential does not answer whether a particular refill may be supplied.
Consider a hypothetical order that passes checkout but cannot be fulfilled after pharmacy review. The payment workflow should identify the charge status, refund owner and customer communication without exposing the clinical reason in unnecessary systems. The example tests responsibilities; it is not a recommendation for a particular authorization or capture sequence.
What should the business explain about fulfillment and information handling?
Identify who sources, stores, dispenses and ships the products, and which team handles a delivery problem or product concern. Describe relevant handling requirements and exception procedures at a business level. A shipping integration or carrier service does not independently establish pharmacy compliance or partner acceptance.
Keep the first processing discussion separate from patient care. Explain systems and responsibilities without sending prescription images, patient names, addresses, diagnosis information or medication histories. If a later review needs evidence, agree the minimum necessary material and the appropriate channel with the receiving party.
Clarify which vendors hold payment information and which hold clinical or dispensing records. A payment provider's security claims should not be treated as a determination that the entire pharmacy workflow meets every privacy obligation. Have the responsible advisers assess the actual arrangement.
What belongs in a pharmacy pre-review summary?
- The dispensing business, operating locations, websites and person responsible for the inquiry.
- Prescribing, dispensing, platform and charging roles described separately.
- Current states served and proposed expansion, with the relevant licensing position.
- Product categories and any compounding or other distinct activity requiring separate assessment.
- Accreditation or certification names, covered entities/domains and current status.
- The point in the order process when payment is requested and how unfulfilled orders are resolved.
- Sourcing, fulfillment, customer support and privacy responsibilities at business level.
- Actual processing activity and clearly labeled plans, together with the specific change needed.
- Required platform or payment functions for confirmation, without patient records or account credentials.
This is suggested preparation, not a complete pharmacy-compliance or underwriting checklist. Ask about the actual supported scope before collecting a large application package or assuming a particular gateway will be available.
What should a potential processing arrangement confirm?
Confirm the named entity, domains, products and markets being assessed. Ask which permissions and credentials the partner requires and how changes in the catalog or operating model should be reported. A favorable discussion about one pharmacy does not establish category-wide acceptance.
Review the complete account proposal, including transaction and software costs, refund handling, contract obligations, funding conditions and any restrictions. Confirm platform compatibility and implementation responsibilities separately. MerchantBridge does not promise approval timing, supported medications, compounding, particular payment methods or settlement terms.
What should an ISO establish before a pharmacy referral?
Discuss an ISO Referral with your own contact details and an anonymized overview. Start by establishing whether the business actually dispenses medication or provides a separate consultation, marketing or technology service. That distinction changes the questions that need to be answered.
- Summarize the dispensing entity, locations, website model and intended customer states.
- Separate clinical fees, pharmacy revenue and any third-party collections.
- Name the actual accreditation programs and their status without treating them as approval.
- Flag compounding or other distinct product activity explicitly for separate review.
- Explain the current payment issue and required functionality.
- Agree permission before a named introduction; exclude prescriptions, patient records and credentials.
MerchantBridge can review potential processing options for the specific profile. The referral should not present existing clinic coverage as pharmacy coverage or promise a placement, compensation arrangement or protected commercial relationship.
